Individual Appointments
Healthy Living NT offers personalised one-on-one education appointments for people with diabetes, pre-diabetes, and heart disease. Our qualified diabetes and cardiac nurse educators and dietitians work with you to address your specific health needs and goals.
Individual appointments are ideal if you prefer personalised attention, have complex health needs, or need flexible scheduling. No doctor's referral is required to book an appointment.
During your individual appointment, our educator will:
- Listen to your health concerns and goals
- Explain your condition in detail
- Discuss medications and lifestyle management
- Answer your questions
- Develop practical strategies tailored to your situation
- Provide resources and support
Appointments typically last 45–60 minutes. You may be offered follow-up appointments to support your ongoing management.
Individual appointments are available in:
- Darwin area – Including suburbs and surrounding regions
- Palmerston
- Alice Springs
If you live outside these areas, phone appointments are available for remote NT residents.
In-Person Appointments – Visit one of our offices for face-to-face education and support.
Phone Appointments – If you're unable to attend in person, or live in a remote area, we offer education by telephone.
Email Support – We can also provide information and resources via email for specific questions.
Individual appointments are suitable for:
- People newly diagnosed with diabetes or heart disease
- People managing complex health conditions
- Those wanting personalised guidance on diet, medications, or lifestyle
- People who prefer one-on-one learning
- Anyone needing flexible appointment times
Family members, significant others, and carers are welcome and encouraged to attend your appointment. They can help you understand your condition and support your health management at home.
Qualified interpreters are available upon request. Please let us know when booking your appointment if you need an interpreter.
